COVID-19 policy for in person appointments

- Please wear a mask to your apt. Mask must cover your nose and mouth.   

- If you currently have or had the following symptoms in the last 14 days, you must let us know: Fever/chills, new /worsening cough, difficulty breathing, sore throat, runny nose (unrelated to seasonal allergies), loss of taste or smell, headache/unexplained tiredness/muscle aches, nausea/vomiting/diarrhea/abdominal pain.
